Requirements of the Social Worker

  • Must have a BSW/MSW Degree then hold relatable experience in the Social Work realm
  • Must have a driver's license, reliable transportation, and car insurance

Responsibilities

The Social Worker will be a key participant in member interdisciplinary teams, weighing in on a host of medical, physical, social, and emotional assessments and Plans of Care. Additionally, this professional will oversee a number of core programming and therapeutic responsibilities, including:

 

  • Overseeing direct social work case management services for caseloads of up to 40 LIFE Center members
  • Providing social service support including screenings, counseling and external referrals, as needed
  • Reevaluating and updating Plans of Care based on change in members’ status, emergent conditions and other relevant circumstances, then communicating changes to all relevant stakeholders (i.e. interdisciplinary teams, families, caregivers)
  • Completing ongoing periodic assessments to ensure a thorough evolution of member care, in keeping with changing needs and requests
  • Documenting all visits, including deviations from approved care plans and prescribed intervention techniques
  • Documenting member medical records and maintaining electronic case management assessments for immediate and future use
  • Identifying development areas, including individual member enhancements and overarching LIFE Center opportunities for growth and expansion, then developing and implementing these improvements
  • Counseling families in both individual and group settings, and leading relevant activities to educate these care participants
  • Visiting in-home members, assessing needs and developing/implementing comprehensive speech/language care plans accordingly
  • General administrative duties, including completing and submitting 1768 documents to required agencies

Qualifications

All candidates must have a Degree from an accredited university, plus one year experience working with the elderly/frail population.
