Senior LIFE is an innovative home and community based Medicare and Medicaid funded program which provides all-inclusive healthcare services and support to seniors living in the community. Senior LIFE makes it possible for seniors to remain at home, enjoying the comfort and reassurance of familiar surroundings while receiving the care they need.  Senior LIFE staff live the mission of the program.  Our team is committed to partnering with our seniors to remain at home through promoting open communication and shared decision making while providing excellent care and services. 

Responsibilities

The OT responsibilities will center on members’ diverse needs and individualized Plans of Care, including:

 

  • Developing occupational treatment plans for all new and existing members, beginning with comprehensive assessments and evaluations and in-depth reviews of overarching Plans of Care
  • Documenting all visits, care activities and changes to member status, to ensure cohesiveness across various departments and interdisciplinary personnel
  • Carrying out periodic assessments to best enhance and expand care plans
  • Evaluating members’ home environments to minimize hazards, and provide additional in-home support, as needed  
  • Educating LIFE Center staff and member families to ensure OT treatment plans are executed when members are in and out of the center
  • Collaborating with interdisciplinary teams and keep all participants informed as to pertinent OT changes as they relate to members’ medical, social or emotional care
  • Working in tandem with the dietary staff to develop adaptive feeding techniques and best practices for members, as needed

 

This position requires an OT with a strong background as well as a desire to provide meaningful service to a diverse elderly population. The OT will be a central figure  in expanding and enhancing Senior LIFE’s mission and; therefore, candidates should be versatile, creative critical thinkers, who can thrive in a team-first environment. Topnotch communication and managerial skills are also vital to ensuring long-term success in this position.

Qualifications

Candidates must be licensed occupational therapists in Pennsylvania, and be committed to participating in the necessary classes/workshops to retain his/her license. All applicants should have at least one year of experience as an OT, specifically working with an elderly population, and demonstrate clinical competency as such. Additionally, candidates with additional management work in rehabilitation or a related medical/social field will be prioritized.
